End of Year Reporting Requirements
Chapters must agree and complete the following yearly requirements in order to maintain their affiliated status. The following forms must be submitted annually with all completed yearly affiliation requirements by December 31.
Required Forms
Fiscal year November 2019 through October 2020.
1. Chapter Leadership Reporting Form
2. Chapter Administration Form
Please have access to the following documents before completing the form.
a. Chapter Articles of Incorporation (if any)
b. Chapter Bylaws
3. Chapter Finances Form
Please have access to the following documents before completing the form.
a. Chapter bank account
b. End of Fiscal Year Financials
c. 990/990N e-postcard receipt
d. Insurance certificates (optional)

Chapter Management Plan
This form is a self-evaluation of the past year's initiatives. Please complete the Excel spreadsheet completing the form.
Chapter Action Plan Form
This form states the goals for the next year along with steps needed to achieve those goals.
Special Reserves Authorization Form (Only if needed)
This form should be submitted if Special Reserves were not requested with end-of-the-year Finances reporting form

HFTP Chapter Scholarship Fund
The purpose of the HFTP Chapter Scholarship Fund is to financially assist HFTP Fully Funded and Partially Funded Chapters experiencing financial hardship from the modified HFTP Annual Affiliation Funding levels for the 2020-2021 fiscal year. Applications must include:
Amount requested, proposed budget for the Funds, and description of the Funds use. Also requested are the Chapter's financial and bank statements for the past year/current year.
