STUDENT CHAPTER AWARD FORM

As an alternative for this year only (2020), HFTP will recognize student chapters for meeting COVID-19 challenges. The traditional Student Chapter of the Year award will not be presented. Student Chapter leaders need to complete the form to be considered for an award this year.

HFTP STUDENT CHAPTER LEADERSHIP & CHAPTER PLAN REPORT FORMS
Due Friday, October 2, 2020

Submit the Student Leadership: Report the chapter Faculty Advisor(s) and student chapter leadership.

2020-2021 academic year

Submit the Student Chapter Plan: The student chapter plan is a tool for student chapters to plan chapter activities based on objectives.  The areas for Annual Reporting include: Summary of Plan, Professional Development, Community and Social Impact, Membership, and the HITEC Student Volunteer Program.

HFTP STUDENT CHAPTER ANNUAL REPORT FORM
Due Friday, March 26, 2021

Submit the Student Chapter Annual Report: The annual report reviews and analyzes the results of chapter activities based on the objectives outlined in the chapter plan. The Annual Report accurately depicts the chapter’s activities including both its successes and failures, during the academic year.
